Medical Records

All patient health care information at MedStar Georgetown University Hospital is confidential. Therefore, medical records cannot be released to any person without the authorization of the patient or the patient’s legally authorized representative (unless authorized by law).

Requesting Medical Records:

All requests for the release of medical records must be submitted in writing and must be dated and signed by the patient or the patient’s legally authorized representative. In the case of a minor, the parent or guardian must sign the authorization.

Submitting a Request:

To submit a written authorization for the release of medical information, fax the completed authorization to 877-544-7752.

GUH has contracted with HealthPort to process medical records requests.

  • First class postage is also applied
  • These prices are for personal patient requests only. All other requests are charged the full state rate.

When are records mailed?

Records are mailed upon receipt of payment—patients should allow 10–15 business days to process the request. All records are mailed via U.S. Mail to the address specified on your request.

How can I obtain a paper copy?

Patients are able to obtain a paper copy of the request form from OB/GYN registration desk (Pasquerilla Healthcare Center, 3rd Floor). To have the medical records request form faxed, please call the OB/GYN Patient Information Line at 202-444-8531.
